Is the menu + ESP internal or overlay (aero etc). Example, if I streamed csgo.exe would the GUIs show or not?

Not necessarily true. A cheat loader can be written in ring3 and have the cheat features load in ring0, it can even have security in ring0 and cheat and loader in ring3...
